Requirements

- Birth Certificate (PSA certified)
- Form 138 / Report Card from previous school
- Good Moral Character Certificate
- School ID or any valid ID of parent/guardian
- 2x2 and 1x1 recent photos
- Baptismal Certificate (for Catholic applicants)
